Salary: £41,554.5 pro rata

As a key people leader with in the service, you will support the operational running of our Day and Night Nursing Team, ensuring patients and their families receive the best possible support.

Job description

Job responsibilities

To lead on the assessment and re-assessment of patients and carers receiving home nursing and volunteer support.

To coordinate and manage the Hospice at Home Day Night Nursing Team, acting as supervisor and co-ordinator for the Day Night Nursing Service in the absence of the Head of Clinical Services.

Assist the Head of Clinical Services to develop and support nurses and the wider team through an effective appraisal and development review process, identifying learning needs and provide training opportunities.

Participate in direct provision of patient care and liaise with members of the multi-disciplinary team and palliative care team in order to achieve this.

  • Clean Driving Licence
  • Ability to travel independently of public transport.

Qualifications

  • Registered Nurse

Desirable

  • Palliative Care Diploma

Skills

Essential

  • Ability to work as an autonomous practitioner and as part of a team.
  • Enhanced communication skills to work with patients, carers and health care professionals.
  • Enhanced care delivery skills with the ability to multi-task and manage a busy and varied workload.

Knowledge

Essential

  • Wide knowledge of medical conditions and symptom management, including medication administration in palliative care.
  • Knowledge of audit.
  • Knowledge of current palliative care agenda both nationally and locally.

Desirable

  • Knowledge of third sector organisations within service area.

Experience

Essential

  • Proven post registration experience and experience of working effectively in a team.
  • Working with palliative care patients and families.

Desirable

  • Community working.
  • Experience in Appraisal process and the training, support and development of staff.
  • Providing clinical supervision to others.
