Referrals to Hospice at Home Carlisle and North Lakeland

Hospice at Home Carlisle and North Lakeland (Hospice at Home) is a charity set up in 1997. We provide free palliative care and support to people with life limiting and their families and carers.

Our main service is our nursing team, made up of Registered Nurses and Healthcare Assistants who provide skilled care during the last year of life and at the end of life. We also have a range of support services that aim to help people live well with their conditions, such as complementary therapy and occupational therapy.

Our care is personalised to each individual. We aim to improve quality of life, provide comfort and dignity at the end of life and support families through bereavement.

We work closely with other health, social care and voluntary organisations to make sure everyone gets the best possible care. Our services cover town and rural locations across 1,500 square miles of North and East Cumbria.

Patients may also self refer

When an individual is referred to our services a member of the team may visit them to introduce the service. In situations where this is not practical, a member of the team will contact the patient or family member/carer to agree a plan of care.
