Hospice at Home’s core service is its skilled team of Registered Nurses and Healthcare Assistants who provide exceptional care and support in people’s last year of life and end of life, also providing additional services such as Lymphoedema, Bereavement and Family Support, Befriending, Occupational Therapy and Complementary Therapy.

We provide individualised care and support to improve quality of life and our end of life care helps facilitate a peaceful and dignified death in the home and support to those who are bereaved.

Our clinical service area covers a mixture of urban and rural communities, a region of approximately 1,500 square miles throughout North and East Cumbria.

Patient Coordinator

Hospice at Home an independent charitable organisation; is looking to recruit to a flexible part time Patient Co-ordinator and admin support worker. The post holder will be able to work flexibly to meet the needs of the Nursing Coordination Team.

The post holder will be responsible for accepting referrals to the nursing service, allocation of patient care and providing general admin support to the clinical teams. The post will also require the individual to communicate with a variety of Health and Social Care Professionals, patients/carers and members of the general public on a daily basis.

Location: Valley Court, Dalston Head Office, with role covering Carlisle, Eden and North Allerdale

Salary: £12.68per hour, £24726 (Pro rata)

Hours: 15 hours

Essential Criteria:

  • Tenacious and resilient approach to a variable workload
  • Administration experience
  • Experience working in health and social care setting

Desirable Criteria:

  • Knowledge of palliative care
  • Knowledge of EMIS
