New Respite Care Service for Neurological Conditions

Hospice at Home is proud to launch a new 12-month nurse-led respite care project, made possible through grant funding. This service is specially designed to support individuals living with progressive neurological disorders and their families.

What we offer:

Up to 5 hours of respite care per visit, tailored to your needs and delivered in the comfort of your own home.

Diagnosis:

Confirmed diagnosis of a progressive neurological disease with in the last 12 months of life.

Progressive Neurological Disorders for example; Parkinsons Disease, Multiple Sclerosis, Motor Neurone Disease, Huntingtons Disease, Progressive Supranuclear Palsy, Multiple System Atrophy, Corticolbasal Degeneration and Myasthenia Gravis.

Clinical Needs:

Requires nursing care that cannot be solely provided by a non-qualified care giver.

May require symptom management, pain relief, medication administration, enteral feeding, or airway management. (Patient specific training may be required prior to commencing the respite blocks)

Respite Requirements:

Identified need for temporary respite support to relieve primary caregivers.

Patient must be able to receive care safely within a home or community setting.

Relevant risk assessments in place if any concerns for providing care within the home e.g. cluttered home, smoker, fire safety.

Palliative and End-of-Life Care:

May be receiving palliative or end-of-life care, requiring specialized nursing interventions.

Consent & Safeguarding:

Patient (or legal representative) must provide informed consent for respite care.

No active safeguarding concerns that would prevent nurse-led respite support.
